Initial Telehealth (Zoom) Appointments

• Full payment is required immediately before the first consultation begins.

• A member of our administrative team will call just before your appointment to process the remaining balance.

• We must also receive your completed registration/consent form before the initial consultation can start.

In-person Appointments (preferred)

• Please allow additional time to locate parking and to complete our intake and registration/consent forms on arrival.
